Seed Exchange-Maternal Health | Ghana Participant Agreement Name * First Name Last Name Email * Phone (###) ### #### Contract This Agreement is entered into by and between REPRA Health (hereinafter referred to as "the Organizer") and the participant (hereinafter referred to as "the Participant") for the "Seed Exchange" Maternal Health Trip to Ghana from July 6-13, 2024. 1. Trip Overview: The Participant agrees to participate in the "Seed Exchange" Maternal Health Trip organized by REPRA Health. The trip will take place in Ghana from July 6-13, 2024. 2. Deposit and Payment: To secure a spot in the trip, the Participant must check the box indicating agreement to the terms and conditions and pay a non-refundable deposit of $750 by the specified deadline. The total fee for double occupancy is $3400, and for single occupancy, it is $3850. The final payment deadline is May 20, 2024. 3. Cancellation and Refund Policy: a) Due to the inherent nature of this excursion, refunds stemming from participant cancellations are strictly prohibited. REPRA Health will exclusively authorize refunds in the event of trip cancellation initiated by the organizers. 4. Trip Inclusions: The total fee includes accommodations, ground transportation, site fees, excursion fees, meals (exclusive of beverages), site tours, a gift bag, and certification. 5. Trip Itinerary and Activities: The Participant acknowledges and agrees to adhere to the trip itinerary provided by REPRA Health. Any deviations from the itinerary must be communicated and approved in advance. 6. Health and Safety: The Participant is responsible for maintaining good health throughout the trip. REPRA Health is not liable for any health issues or accidents that may occur during the trip. REPRA Health is not liable for extended stays in Ghana if the Participant is unable to travel due to Covid-19 or other medical condition. While Ghana is generally a safe country, REPRA Health is not liable for any personal liabilities, including but not limited to pickpocketing, theft, or other personal property losses incurred during the course of the Seed Exchange- Maternal Health Trip. 7. Code of Conduct: The Participant agrees to conduct themselves in a respectful and considerate manner towards fellow participants, local communities, and trip organizers. Disrespectful and inconsiderate behavior includes but is not limited to use of profanity, fighting, illegal behaviors or other behavior deemed inappropriate by program coordinators. Any violation of this code may result in the Participant's removal from the trip without a refund. 8. Travel Insurance: The Participant is required to have comprehensive travel insurance covering medical emergencies, trip cancellations, and other unforeseen events. Proof of insurance must be provided to REPRA Health before the trip start date. 9. Travel Documents: The Participant is responsible for ensuring that all required travel documents, including passports, visas, vaccination records are valid and up-to-date. REPRA Health is not responsible for missed flights, or lack of documentation barring entry into Ghana. 10. Photography, Media and Email Release: The Participant grants REPRA Health the right to use photographs and videos taken during the trip for promotional and marketing purposes. The participant also grants REPRA HEALTH to share their email contact to carefully selected partners and other participants. 11. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of Maryland, USA.
